Ralph the releaser

Track changes between tags.

Handles the following for you:

* Shows you the commits on master since the last tag.
* Updates a CHANGELOG.md file.
* Creates a package contianing all your npm dependancies so they can be installed offline.
* Creates a git tag with an NPM shrinkwrap file so you can replicate from a git tag
